LSR7 hosts sports physical clinic for student athletes
LSR7 is offering a physical clinic in July so parents can get their student athlete the annual physical required to practice and compete for the 2026-27 school year.
When: July 21st, 2026
Where: Lee's Summit North High School (East Entrance-Fieldhouse Entrance)
Time: 4 - 7 pm
Who: LSR7 students entering Grades 6th-12th for the 2026-2027 school year

Also, please remember your student needs to JOIN A TEAM so that your student’s profile is visible to his/her school activities office.
The Physical Form will be valid for two years from the date of the exam (unless noted otherwise by your physician).
Parents will need to complete the registration process through Final Forms each year. This process includes providing current insurance coverage information.
The LSR7 School District partners with FinalForms, an online forms and data management service to allow you to complete and sign athletic participation forms for your students. FinalForms saves data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice. FinalForms also pre-populates information wherever possible, for each of your students, saving you time. You may review your data at any time to verify it is current. You will be required to sign your forms once per year and after any update.
